I'm going to tell you what the S, the D, the A, and R all stand for, and show you various examples of how to chart in this format. There are different scenarios where you would use different parts of the F dar format when charting, so I want to simplify it for you and help you be prepared for when you're in clinicals or whenever you're actually working at your new job. So first, let's talk about what it is. Like I said at the beginning, this is the most commonly used format in hospitals or other healthcare settings. When I first started out nursing, we used the SOAP format, but a lot of hospitals are now turning to this FDR format. What it does is, it's also used by other disciplines alongside nursing. So when you're looking through a patient's chart in the progress notes, you may see not only nursing documentation, but also documentation from respiratory therapy, occupational therapy, physical therapy, and nutrition. We all document together in this nice uniform format that we all understand and know how to read.
